 Registration Information To enroll a student for the first time, a parent or guardian needs to provide a certified copy of the child's birth certificate, immunization records, emergency contact name & number, child's doctor's name & number and proof of residence (rental/lease agreement or purchase/escrow agreement or property valuation statement OR copy of utility bill with physical address).
